Civil Rights Training with Certificate of Completion
For organizations distributing TEFAP or CSFP (senior food boxes), the United States Department of Agriculture requires that any agency staff or volunteers who interact with people served, even one-time volunteers, complete the Civil Rights Training EACH YEAR. Connecticut Foodshare partners need to keep a record of when each staff or volunteer completed their Civil Rights Training.

2. Once you have read the training document, please complete this form.
3. Upon completion of this form, a Certificate of Civil Rights Training will be emailed to you as a PDF and the Program Coordinator if you provide their email on this form or you can forward a copy to the appropriate person at your organization.
